Where the sources disagree

Most LDL-C values on consumer panels are estimated using an equation, not directly assayed, and the equations become unreliable when triglycerides are high or LDL is very low. This matters for a specific reason: when a platform advertises a biomarker count, calculated values like this one are frequently counted alongside directly measured ones. If your LDL-C looks implausible next to your apoB, the equation is the likely explanation.

The strongest case for it

Cumulative lifetime LDL exposure drives atherosclerosis, and trials in higher-risk groups show benefit continuing well below conventional targets.

What it leaves out

Trial evidence for this threshold comes from people at elevated baseline risk. Extrapolating it to everyone converts a risk-stratified decision into a universal product recommendation.

Grade B: Consistent observational association with hard outcomes, but the specific cutoff is extrapolated rather than trial-tested.

  • Commonly calculated rather than measured; accuracy degrades at high triglycerides.
  • Where LDL-C and apoB disagree, apoB is the better risk predictor.
  • Requires fasting for the standard calculation to be valid.
