Where the sources disagree

HOMA-IR is the clearest illustration of why biomarker counts are misleading. It is arithmetic performed on two other results, yet it commonly appears as a separate line item on a panel that advertises '100+ biomarkers'. You did not receive a third measurement. Combining fasting glucose and insulin captures more information than either alone, and lower values do track with better insulin sensitivity.

What it leaves out

HOMA-IR is calculated, not measured. It inherits every problem of the insulin assay it depends on, and it was developed for comparing populations in research, not for grading individuals against a target.

Grade C: Association is real but confounded, assay-dependent, or the intervention trials that would confirm it are null or missing.
